1. CLOSE CONTACT CALLS: If you get the call that your son was close contacted it becomes the family’s decision on what to do. If they are without symptoms, then they can continue with school and playing. TSSAA has given us a form that has to be signed for each time you get that call. It’s just the parents acknowledging they were notified about close contact, your son is asymptomatic, and they plan to continue forward with school/sports. If you are contacted for close contact and your player does not have any symptoms at all, use this form, Click Here, for permission to continue participation.
2. The other form is if they are actually covid positive. It’ll have a place for the Dr.’s signature, a return date, and how they recommend the player assimilating back with the team. Meaning: right back in full speed or work them in. Click Here for the form.
3. When any of the above applies to you, you can send it in via phone image to my email, scanned to my email, or print and bring a hard copy. Any way is fine.
This will be in effect for any new positives or close contact calls starting today, 8/23/21.
Please continue to monitor your student for fever, cough, body aches, sore throat, and runny nose. Keep them at home if any symptom presents itself.

Fill out and print the Preparticipation Physical History and Evaluation Form to take with you to the doctor.
Follow instructions to create and maintain a Final Forms Account for each athlete.
**Every athlete must complete these steps prior to participation in their sport.
Physicals are good for 1 school year and not 1 calendar year. Everyone must obtain a new physical each year after April 15th.
Upload Physcials and PPE forms to final forms and sign all documents (players and parents in final forms)
